MRSA陽性であった兎眼性角結膜炎のポビドンヨードによる治療経験

Use of povidone-iodine solution for a case of MRSA-positive keratoconjunctivitis with lagophthalmos

　37歳男性の右眼に兎眼性角結膜炎が7年前に発症した。その1年前に交通事故による頭部外傷があり，右眼は眼球癆になっていた。過去7年間にレボフロキサシンとオフロキサシンの点眼を受けていた。当科初診時の視力は右0，左0.7であった。結膜培養でメチシリン耐性黄色ブドウ球菌（methicillin-resistant Staphylococcus aureus：MRSA）が分離された。0.63％ポビドンヨード液の点眼を1日4回行った。その1か月後にMRSA陰性になり，眼球内容除去術と眼瞼縫合術を行った。さらに2か月間ポビドンヨードの点眼を行い，義眼を作製し，経過は良好である。

　A 37-year-old male developed phthisis bulbi and lagophthalmic keratoconjunctivitis 7 years ago. He had been involved in traffic accident with head trauma in the previous year. He had been continuously treated by topical levoxacin and ofloxacin during the past 7 years. Methicillin-resistant Staphylococcus aureus（MRSA）was isolated from the conjunctival discharge. We treated his right eye by 0.63％povidone-iodine ophthalmic solution 4 times daily. The conjunctiva became free of MRSA one month later. He is doing well after exenteration and tarsorrhaphy followed by wear of false eye.
