オフロキサシン耐性Corynebacteriumによる周辺部角膜潰瘍の1例

A case of marginal corneal ulcer due to ofloxacin-resistant Corynebacterium

　65歳女性が1か月前からの右眼痛で紹介され受診した。約2年前から片眼または両眼の痛みが繰り返していた。そのつど角膜潰瘍と診断され，オフロキサシン，フルオロメトロン，ベタメタゾンなどの点眼で短期間に治癒していた。今回の受診時に，右角膜の2か所に浅い周辺部角膜潰瘍があり，全周に角膜浅層の混濁と血管侵入があった。オフロキサシン点眼とプレドニゾロン内服は無効であった。オフロキサシン耐性のCorynebacteriumが角膜擦過物から培養された。1日6回のセフメノキシム点眼に変えたところ，1週間後に潰瘍は治癒した。角膜潰瘍の治療では，耐性菌による可能性があることを示す症例である。

A 65-year-old woman presented with pain in her right eye since one month before. She had had episodes of uni-or bilateral ocular pain during the past 2 years. Under diagnosis of corneal ulcerat each occasion,the condition had cured with instillation of ofloxacin,fluorometholone,or betamethasone. Her right cornea had two lesions of shallow marginal ulcer. Corneal opacity with neovascularization was present along the whole circumference. Initial treatment with topical ofloxacin and systemic prednisolone proved futile. Ofloxacin-resistantCorynebacteriumwas then isolated from corneal scrapings. The ulcer was brought to cure one week after treatment was switched to instillation of cefmenoxime 6 times daily. This case illustrates that drug-resistant microorganism may be involved in the pathogenesis of corneal ulcer.
