後眼部より睫毛が発見された特異な眼外傷の1例

Cilia in the posterior eye segment in a case of penetrating ocular injury

　18歳男性が作業中，左眼に径2mmの鋼線が刺入した。その直後から眼痛と出血，視力低下があり受診した。左眼角膜の上耳側から中央部に及ぶ裂傷があった。前房出血があり，中間透光体と眼底は透見できなかった。手術の当日に角膜を縫合し，前房を洗浄した。術後のX線とCT検査で異物は発見されなかった。受傷から2週間後に超音波検査で硝子体出血と網膜剝離が推定された。白内障手術と硝子体切除術を行い，硝子体腔内に2本，網膜下の血腫内に1本の線状異物が発見された。異物は3本とも黒色であり，長さが5ないし7mmで，1本には毛根があった。これら異物はすべて受傷時に迷入した睫毛であると判定された。以後の経過は良好で，0.5の最終視力を得た。

A 18-year-old male was hit by a piece of steel wire in his left eye while working. He visited us because of pain,hemorrhage and impaired vision. His left cornea was ruptured extending from the limbus to the apex. Posterior eye segment could not be examined because of hyphema. The corneal wound was sutured followed by irrigation of the anterior chamber. Postoperative X-ray and CT examinations showed no intraocular foreign body. Two weeks later,echography showed finding suggestive of vitreous hemorrhage and retinal detachment. During ensuing vitreous surgery,two linear foreign bodies were found in the vitreous and another within subretinal hematoma. All were black in color and were 5 to 7mm in length. One piece had hair root at one end. All the 3 foreign bodies were identified as cilia that emigrated into the eyeglobe during ocular injury. Later findings were uneventful and his left eye regained visual acuity of 0.5.
