経口抗癌剤エルロチニブによる薬剤性ぶどう膜炎が疑われた1例

A case of uveitis following treatment with peroral erlotinib

要約　目的：エルロチニブの経口投与後に発症した両眼性前部ぶどう膜炎の症例の報告。症例：85歳男性が両眼の霧視と充血で紹介受診した。15か月前に非小細胞肺癌と診断され，1次と2次の化学療法を受けたが病巣が拡大した。6週間前から3次治療として1日量150mgのエルロチニブの経口投与が開始された。10日の休薬後に内服を再開したが，4日前に霧視と充血が生じた。所見：矯正視力は右0.8，左1.0で，両眼の瞳孔縁に結節形成，右眼に虹彩後癒着があった。副腎皮質ステロイドの点眼2日後には病像に変化がなく，エルロチニブの投薬中止から4日後に虹彩炎症状はほとんど消失した。結論：本症例では，肺癌による全身病変があり，エルロチニブの投与が加わってぶどう膜炎が生じた可能性がある。

Abstract.　Purpose：To report a case of bilateral anterior uveitis that developed after treatment with peroral erlotinib. Case：An 85-year-old male was referred to us for blurring and hyperemia in both eyes. He had been diagnosed with non-small cell lung carcinoma 15 months before. The lesion failed to respond to two sessions of chemotherapy. He started receiving peroral erlotinib at the dail dosis of 150mg since 6 weeks before. Findings：Corrected visual acuity was 0.8 right and 1.0 left. Both eyes showed Koeppe nodules. The right eye showed posterior synechia. Signs of anterior uveitis remained unchanged 2 days after topical corticosteroid. Withdrawal of erlotinib was followed by remarkable improvement of uveitis 4 days later. Conclusion：Systemic erlotinib may have triggered uveitis in both eyes after longstanding systemic state of inflammation by lung carcinoma.
