眼病変を合併したChurg-Strauss症候群の2例

Two cases of Churg-Strauss syndrome with ocular involvement

目的：Churg-Strauss症候群に眼病変が併発した2症例の報告。症例：1例は46歳女性。気管支喘息，多発性神経炎，眼底出血の既往があった。初診時に両眼の新生血管を伴う閉塞性網膜血管炎，高IgE血症，好酸球増加があった。3か月後に両眼に肉芽腫性ぶどう膜炎，7か月後に網膜血管炎の再燃，19か月後に右網膜中心静脈切迫閉塞，2年後に左眼硝子体出血と血管新生緑内障が発症した。ステロイド内服と手術で軽快した。他の1例は40歳男性。アレルギーの既往はない。初診時に右眼眼窩内に多発性腫瘤，両側上顎洞炎，肺病変，高IgE血症，好酸球増加があり，ハウスダストIgE強陽性であった。眼窩腫瘤の生検で好酸球の著明な浸潤と血管炎の所見があった。ステロイド内服で眼・肺・副鼻腔病変が軽快した。結論：Churg-Strauss症候群では，喘息などのアレルギー疾患が先行するか否かにかかわらず，眼病変が発症することがある。

Purpose：To report two cases of Churg-Strauss syndrome with ocular involvement. Cases and Findings：A 46-year-old female had had bronchial asthma,polyneuritis,and retinal hemorrhage. When seen by us,she had occlusive retinal vasculitis with neovascularization,elevated IgE in the blood,and eosinophilia. She later developed granulomatous uveitis,impending occlusion of central retinal vein,and neovascular glaucoma. Ocular lesions regressed following surgery and peroral corticosteroid. Another 40-year-old male was free of allergic diseases. He presented with multiple tumors in the right orbit,bilateral parasinusitis,pulmonary lesion,elevated IgE and eosinphilia. He was highly positive for housedust IgE. Biopsy of the tumor revealed a marked infiltration of eosiophils and angiitis. Lesions in the eye,lung and paranasal sinus regressed following peroral corticosteroid. Conclusion：Ocular lesions in this syndrome may develop irrespective of preceding allergic diseases including asthma.
