炭酸脱水酵素阻害薬点眼後に不可逆的な角膜浮腫をきたした1例

A case of persistent corneal edema following topical instillation of carbonic anhydrase inhibitor

82歳女性が12年前に白内障と開放隅角緑内障の同時手術を受けた。7年前に右眼の眼圧が不安定になり，β遮断薬を点眼していた。両眼とも視力は正常で，眼圧は右眼21mmHg，左眼18mmHgであった。右眼に炭酸脱水酵素阻害薬であるブリンゾラミド点眼を1日2回開始した。その1週後から点眼直後に右眼の霧視を自覚していた。点眼開始から1か月後の再診時に，右眼視力は矯正0.5，右眼眼圧は12mmHgであり，右眼の角膜に上皮浮腫，実質混濁，デスメ膜の皺襞形成があった。角膜内皮細胞密度は，右眼1,161/mm²，左眼1,312/mm²であった。ブリンゾラミド点眼をやめ，さらに2か月後にほかの眼圧降下薬の点眼を中止した。点眼中止から12週後まで，角膜上皮と実質の浮腫が続き，自覚症状と視力は回復していない。12年前の内眼手術で角膜内皮細胞数が減少し，さらに炭酸脱水酵素阻害薬の点眼が残存角膜内皮の機能を障害したことが，角膜浮腫の原因になったと考えられる。

An 82-year-old woman had received surgery for cataract and open-angle glaucoma 12 years before. Intraocular pressure（IOP）became unstable 7 years before,necessitating instillation of beta-blocker in her right eye. Her visual acuity remained 1.0 in either eye. IOP was 21 mmHg right and 18 mmHg left. She then started additional instillation of brinzolamide twice daily. She noticed,reportedly,blurring of vision one week after instillation. When seen one month after brinzolamide,her right eye showed corneal edema,stromal opacity and folds in Descemet membrane. IOP in the right eye was 12 mmHg. Corneal endothelial cell density was 1,161/mm² right and 1,312/mm² left. Corneal edema persisted at least for 12 weeks after discontinuation of all topical medications. It appeared that previous intraocular surgery induced decrease in endothelial cell population. Additional instillation of brinzolamide,a carbonic anhydrase inhibitor,triggered dysfunction of corneal endothelium with resultant corneal edema.
