サイトメガロウイルス網膜炎発症を契機にHIV感染症が発見された1症例

Cytomegalovirus retinitis as the initial manifestation in a case of human immunodeficiency virus（HIV）infection

54歳男性が両眼の網膜炎で紹介され受診した。8か月前に帯状疱疹を発症し，微熱が続き，極端な体重減少があった。過去5年間に中国とタイへの渡航歴があった。矯正視力は右眼0.2，左眼0.03であり，両眼に前部ぶどう膜炎，硝子体混濁，眼底の黄色白色滲出斑と血管炎があった。血漿と房水のサイトメガロウイルス（CMV）-DNAが著明に増加し，全身検索でヒト免疫不全ウイルス（HIV）抗体が陽性であった。HIV網膜炎と診断した。汎血球減少があり，骨髄抑制がないとされるホスカルネットによる抗CMV療法を行ったのち，HIVに対するhighly active antiretroviral therapy（HAART）を開始した。抗CMV療法は3週後にガンシクロビル内服に切り替えた。血中HIVウイルス量が減少し，1か月後に測定限界以下になった。CMV網膜炎は鎮静化し，発症から12か月後の現在まで再発はなく，右眼0.3，左眼0.1の矯正視力を維持している。CMV網膜炎を契機としてHIV感染が発見され，治療が奏効した症例である。

A 54-year-old male was referred to us for bilateral retinitis. He had had herpes zoster 8 months before,followed by slight fever and weight loss. His corrected visual acuity was 0.2 right and 0.03 left. Both eyes showed anterior uveitis,vitreous opacity,retinal exudates and vasculitis. Cytomegalovirus-DNA was positive in the serum and aqueous. He was positive for human immunodeficiency virus（HIV）antibody. He was treated by systemic foscarnet for 3 weeks followed by highly active antiretroviral therapy（HAART）. CMV retinitis subsided with visual acuity of 0.3 right and 0.1 left during 12 months until present. This case illustrates that CMV retinitis may be the initial clinical manifestation of HIV infection.
