　膝関節前十字靱帯再建術後のガーゼ遺残により発生した異物肉芽腫を経験した．症例は61歳の女性で，右膝関節外側から膝窩部の腫瘤を主訴に当院を受診した．20年前に腸脛靱帯を用いた右膝関節前十字靱帯再建術を受けていた．針生検で採取した組織では慢性炎症を示す所見が得られたため，既往の手術に関する反応性病変を疑った．全摘出術を施行したところ，腫瘤内から遺残ガーゼが見出され，組織学的にも異物肉芽腫であることが確認された．膝関節以遠部位での発生，前十字靱帯再建術後，いずれも異物肉芽腫としては非常に稀な症例である．

　We report a case of gossypiboma around the knee joint caused by a surgical sponge left behind during surgical reconstruction of the anterior cruciate ligament (ACL).　A 61-year-old woman, with a history of ACL reconstruction 20 years before complained of a mass in the lateral and popliteal region of the right knee joint.　Radiological examination and histological examination of a biopsy specimen revealed that the mass consisted of reactive tissue and chronic inflammation.　We excised the mass, and a surgical sponge was discovered inside it.　Reports of gossypibomas around the knee joint and after ACL reconstruction surgery have been rare.
