縦皮切による弾発指手術

Longitudinal Skin Incision for Surgery of Trigger Finger

　われわれは麻生の評価を用いて，縦皮切による弾発指手術後の瘢痕に対する患者の評価を行った．対象は母指以外の弾発指43例51指で，平均63.3歳，平均観察期間は8.2カ月で，指別では示指8指，中指23指，環指17指，小指3指であった．瘢痕の評価は，優：37指73％，良：14指27％，可，不可はみられなかった．瘢痕拘縮による指の伸展障害やそのほかの合併症は認めなかった．一般に縦皮切による切開線は，術後瘢痕拘縮を来すことから禁忌とされているが，今回の調査結果から縦皮切は選択されてよい皮切であると思われた．

　Transverse incision is the standard incision for surgery of trigger fingers to prevent postoperative extension impairment by the skin scar. The purpose of this study is to evaluate the clinical results of the longitudinal skin incision in surgery. During 2002 through 2007, 51 fingers of 43 patients were treated surgically. Eighteen male and twenty-five female, aged from 37 to 78 years (averaged in 639 were involved. The affected digits were 8 index, 23 middle, 17 ring, and 3 little fingers. Cases of trigger thumb are excluded as we treated by transverse incision. Clinical result of scar formation at an average follow-up in 8.2 months, based on Aso's classification, showed excellent in 37 fingers and good 14 fingers, and none was in fair or poor. No extension impairment by the skin scar and no other complications were seen. Longitudinal skin incision might be a valuable incision for the surgery of trigger fingers.
