　抄録：脊髄内に発生する神経鞘腫の報告は少ない．今回われわれは胸髄軟膜と根糸の境界部に発生し脊髄軟膜下に発育したと考えられた神経鞘腫を経験したので報告する．症例は49歳，女性．患者は2002年2月末より背部痛が出現し，4月頃より両下肢のしびれ感，右下肢の脱力による歩行障害のため，同年7月12日当科を受診した．MRI，CTM，ミエログラムにて第1胸椎下縁から第2胸椎椎体レベルの硬膜内髄外病変が疑われ，CTMでは髄表病変の可能性も示唆されたため，同年8月7日，顕微鏡下摘出術を行った．病変は尾側に後根糸を含んでいたが，脊髄との境界面は不明瞭で病変より頭側の根糸は確認できなかった．摘出標本は神経鞘腫であった．本症例は脊髄軟膜と後根糸の境界部のSchwann細胞から発生し軟膜下に進展，発育したものと考えられた．また術前検査において，腫瘍と脊髄との局在関係の評価でCTMが有用であった．

　Intramedullary schwannoma with no evidence of neurofibromatosis is very rare. We report extramedullary schwannoma had grown into the spinal cord. It was impossible to differentiate this tumor from other intramedullary tumors and difficult to preoperatively know its exact relation to the spinal cord by MRI. We almost could suppose its relationship between the tumor and the spinal cord by CTM. We think the tumor was born from dorsal-root Schwann cells in a “critical area” where posterior roots lose their sheaths on entering the pia mater, and grew into the spinal cord.
