はじめに

　近年の化学療法の導入と，外科的手術手技の進歩により骨肉腫患者の機能的予後，あるいは生命予後は劇的に改善した．その一方，約40％の骨肉腫患者は原発巣の根治手術後に肺転移が生じ予後不良な経過をたどる．肺転移が生じた場合には化学療法剤に対して抵抗性を示すことが多く，その制御ははなはだ困難となる．骨肉腫の予後をさらに向上させるためには，①原発巣に対する根治手術後に肺転移巣が出現しやすい骨肉腫患者を明らかにすること，②原発巣根治手術後に肺転移巣が発現増大する機序を明らかにすることが問題解決への糸口を与えることになる．

　Folkmanら^6）が癌の増殖や転移巣形成の際の血管新生の重要性を提唱して以来，胃癌，食道癌，子宮癌，腎癌など様々の癌腫で増殖と進展における血管新生の関与が研究されている．1998年以来，われわれも骨肉腫肺転移成立に血管新生が関与しているとの仮説を立てて研究を行ってきた．現在までに明らかになった知見をふまえ，骨肉腫肺転移成立における血管新生の役割を解説し，骨肉腫に対する新たな治療戦略を考えてみたい．

　We investigated the clinical significance of angiogenesis in the establishment of pulmonary metastases of osteosarcoma. The metastatic rate of the patients who had VEGF-positive tumors was significantly higher than that of those who had VEGF-negative tumors. The serum VEGF levels were higher in patients who relapsed with pulmonary metastases than those of patients without relapse. We also demonstrated in an experimental animal models that removal of the primary osteosarcoma tumor acclerated the progression of pulmonary metastasis through systemic angiogenic activation. These results suggest that the establishment and progression of pulmonary metastases of osteosarcoma are strongly associated with angiogenesis. Anti-angiogenic should improve the prognosis of osteosarcoma patients.
