　抄録：変形性膝関節症（以下，膝OA）を合併した両膝樹枝状脂肪腫の1例を経験したので報告する．症例は73歳の男性である．20年前より，誘因なく両膝の腫脹と軽度の痛みが出現し，15年前より，膝OAの診断で保存的に加療されていた．徐々に痛みが増強したため，人工膝関節全置換術を施行した．同時に滑膜切除を施行した．病理診断は樹枝状脂肪腫であった．本症は，長期罹患により，2次性のOAを惹起する可能性が指摘されている．本症は，臨床症状とMRI所見に特徴があり，早期に診断して切除することが望ましい．

　Lipoma arborescens is a rare condition, and its most common site is the knee. We performed total knee arthroplasty and synovectomy in a patient with bilateral lipoma arborescens of the knee associated with severe osteoarthritis. If lipoma arborescence is borne in mind, the correct diagnosis can be made on the basis of its characteristic clinical and MRI findings. Because there is evidence that long-standing lipoma arborescens can lead to secondary osteoarthritis, early diagnosis and resection are necessary.
