　抗リン脂質抗体症候群は，リン脂質に対する抗体を有し血栓症を中心とする様々な症状を呈する疾患群で，多発性，再発性の脳虚血病変を高率に合併することが知られており，原因の明らかでない脳虚血症状を呈した場合，その検索を考慮すべき疾患である。今回われわれは，中大脳動脈高度狭窄にて明らかになった，抗リン脂質抗体症候群に合併した未破裂脳動脈瘤の1千術例を経験した。虚血性病変を合併した未破裂脳動脈瘤の手術治療には，脳の脆弱姓に配慮した対応が必要であるが，本症例では術前よりの抗痙攣剤の使用，間欠的脳圧排操作，術中SEPモニタリング，術後の抗凝固薬の使用などの周術期管理を施行した。抗リン脂質抗体症候群に合併した未破裂脳動脈瘤の千術も，厳密な対応により安金に施行可能であると考える。

Anti-phospholipid antibody syndrome (APS) is char-acterized by the presence of antibodies to phospholip-ids and various symptoms including those derived from thrombosis. APS has been shown to be fre-quently accompanied by multiple and recurrent cere-bral ischemic lesions, suggesting that APS may play a critical role in developing cerebral ischemia of un-known etiology. Here, we present a case with an un-ruptured cerebral aneurysm accompanied by a severe stenosis of the middle cerebral artery as a manifesta-tion of APS.
