小脳出血の血腫除去術後にCerebellar mutismを生じた成人の1例

An Adult Case of Cerebellar Mutism after Removal of Cerebellar Hematoma

　小児の小脳腫瘍の摘出後にcerebellar mutismを呈する例があることは比較的よく知られている。しかし，成人の小脳出血の術後にcerebellar mutismを呈した例はこれまで1例が知られているにすぎない。われわれは64歳の男性で高血圧性小脳出血の血腫摘出後にcerebellar mutismを呈した1例を経験した。血腫は右側歯状核と右上小脳脚の部分を中心として存在し，小脳虫部には及んでいなかった。また術後のSPECTで左側側頭葉から頭頂葉の血流の低下が認められた。本症例のcerebellar mutismの発生機序としては歯状核視床皮質路において歯状核と上小脳脚の部分が損傷し，対側皮質の補足運動野を中心とする部分の血流が低下してmutismが発症した可能性が示唆された。

Cerebellar mutism is a well documented complica-tion of posterior fossa tumor surgery in children. But only one case presenting with cerebellar mutism after removal of a cerebellar hematoma in adults was re-ported previously. We present 64-year-old male devel-oping cerebellar mustism after surgery for hyperten-sive cerebellar hemorrhage. A cerebellar hematoma was located in the right side dentate nucleus and cere-bellar peduncle dominantly not extending to the cere-bellar vermis.
