　静脈性虚血を主徴とした硬膜動静脈奇形（DAVM）3例の臨床経過を報告し，静脈性虚血を主徴とするDAVMの病態と臨床的特徴について検討した。静脈洞の閉塞性変化を合併した後頭蓋窩DAVMは，静脈還流が直静脈洞に集中することにより，大脳白質や基底核の静脈性虚血をきたす。痴呆症状やそれに続発する意識障害などの進行性の神経症状を呈し，CT／MRI上，多発性の脳実質病変と異常に拡張・蛇行した多数の静脈を認めることが特徴である。

We report three patients with dural arteriovenous malformation (DAVM) in the posterior fossa presenting the venous ischemia and the patho-physiology and the clinical characteristics of DAVM presenting the venous ischemia are discus-sed. Associated with occlusive changes in the venous sinuses, DAVM in the posterior fossa develops the venous hypertension of the straight sinus and the venous ischemia of cerebral white matter and basal ganglia. The venous ischemia presents progressive symptoms consisting of dementia with or without the ensuing consciousness disturbance.
