頭蓋内出血で発症した前頭蓋窩硬膜動静脈奇形の1例

A Case of Dural Arteriovenous Malformation in the Anterior Cranial Fossa Presenting with Intracranial Hemorrhage

　今回我々は，頭蓋内出血で発症した前頭蓋窩硬膜動静脈奇形の1例を経験したので報告する。症例は60歳の男性で突然の頭痛，意識障害で発症した。脳血管撮影上，両側の前篩骨動脈を流入動脈，皮質静脈を導出静脈とし，皮質静脈の一部は嚢状に拡張して上矢状洞に流入していた。明らかなnidusは描出されなかった。左前頭開頭による摘出術を行い，治癒せしめた。皮質静脈の嚢状に拡張した部分は脳表と強く癒着しており，ここが出血源と考えられた。前頭蓋窩硬膜動静脈奇形は従来稀な疾患と考えられていたが，近年その報告が増えてきている。我々が調べ得た具体的記述のある58例について，特徴を検討した。我々が経験した症例は臨床的にも画像的にも前頭蓋窩硬膜動静脈奇形の典型例であると考えられた。

We report a case of dural arteriovenous malfor-mation (dAVM) in the anterior cranial fossa presenting with intracranial hemorrhage. The patient was a 60-year-old man who became uncon-scious shortly after developing an acute headache. Computed tomography performed on admission demonstrated a left frontal intracranial hematoma together with subarachnoid hemorrhage and a sub-dural hematoma.
