Abstract 文献概要

　脳梗塞の急性期進行例の臨床上の特色について検討した。方法：対象は発症当日入院の内頸動脈領域梗塞例309例である。結果：①症状進行は71例（23.0％）でみられ，60例は完成梗塞例，11例はRIND例である。②進行例は非進行例に比し高齢である（p＜0.05）。③皮質枝域群および全中大脳動脈域群における進行例は穿通枝域群に比し多い（p＜0.001）。④進行は第5病日までに全例確認でき，第2病日が39例，第3病日が13例である。⑤進行の停止は進行1日後が最多で，最長7日後までみられた。⑥進行例は入院時に高次脳機能障害のみの11例にはみられず，知覚，運動障害のみの187例では8例（4％），知覚，運動障害に意識障害および高次脳機能障害を伴う111例では63例（57.0％）みられた。⑦進行例は心房細動例に多い（p＜0.001）。⑧71例中56例で高度の上肢運動機能障害が残存した。結論：入院時の意識，高次脳機能障害および心電図上，心房細動の有無についての検討が重要である。

Diagnosis of deterioration of neurological deficits in the early stage after hospital admission immedi-ately after the onset of cerebral infarction is impor-tant in establishing a treatment plan. This study investigated the clinical characteristics of progres-sive stroke on admission. The subjects were 309 patients admitted on the day of the onset of symp-toms and showed a low density area on CT images within 5 days.
