硬膜外膿瘍により急性横断性脊髄障害を呈した多発性骨髄腫の1例

A Case of Multiple Myeloma Complicated by Acute Transverse Myelopathy due to Epidural Abscess

　51歳男性。発熱と腰痛に続いて第9胸髄レベル以下の感覚障害，両下肢麻痺，膀胱直腸障害が急速に進行。諸検査から硬膜外膿瘍による急性横断性脊髄障害と診断した。脊髄MRIでは胸腰髄の傍脊柱筋内に多発性の膿瘍形成を伴っていた。またM蛋白を伴う高ガンマグロブリン血症が持続的にみられ基礎疾患としてDurie-Salmonの病期分類でStageⅠの多発性骨髄腫の存在が明らかになった。多発性骨髄腫では骨病変の進展に先立って種々の免疫抑制機序が出現し得ることが報告されており，本例で膿瘍が急速に拡大した原因としてこれらの免疫抑制機序が関与している可能性が考えられた。誘因なく発症した硬膜外膿瘍の基礎疾患として多発性骨髄腫は重要であるばかりでなく，本症では病初期から免疫不全をきたして重篤な感染症を惹起する可能性があり注意が必要である。

A 51-year-old man developed fever and lumbago followed by rapidly progressive bilateral sensory disturbance below the 9th thoracic spinal cord level, flaccid paraplegia, urinary obstruction and constipa-tion. Based on radiological examinations and labo-ratory findings, a diagnosis of transverse myelopa-thy due to epidural abscess was made. A series of MRI studies revealed multiple abscess formation in the paravertebral muscles.

Hypergammaglobulinemia with M protein was observed continuously, and further examination revealed multiple myeloma in the early stage.
