　脊椎硬膜外膿瘍と診断された31例を，単独に発症した硬膜外膿瘍(単独群)と化膿性脊椎炎に併発した硬膜外膿瘍(併発群)に分け比較検討した．患者背景，初発症状，血液検査所見は両群に差がなかった．単独群では併発群に比べて麻痺の発生率が有意に高く，症状を自覚してから麻痺が発症するまでの期間が短い傾向にあり，さらには全例に手術治療が必要であった．単独群では病期に関わらず手術治療を選択してもよいと考えられた．

　We conducted a clinical review of cases of spinal epidural abscess (SEA) and divided then into cases of SEA alone and cases complicated by pyogenic spondylitis.　There were no differences between the two groups in patient background factors, primary symptoms, or blood examination findings.　SEA alone was associated with a significantly higher incidence of paralysis, and the interval between the diagnosis in the cases of SEA alone and the development of paralysis was shorter.　Surgical treatment should be considered in cases of SEA alone regardless of the stage of the disease.
