テント髄膜腫に合併した脊髄空洞症の1例

Syringomyelia Associated with Tentorial Meningioma

　テント髄膜腫に脊髄空洞症を合併した1例を経験した。患者は28歳男性で，左上肢のしびれと歩行のふらつきで当方に入院した。神経学的に軽度の躯幹失調，左dysmetria，左上肢のdysesthesiaが認められた。CT, MRIで左後頭蓋窩に石灰化を有する最大径6cmのテントに接する腫瘍性病変，小脳扁桃ヘルニア，C2上縁からTh3に至る脊髄空洞が描出された。テント下面に発生した髄膜腫の診断で後頭下開頭で腫瘍を全摘した（Simpsonのgrade2）。術後のMRIでは腫瘍は全摘され，扁桃ヘルニア，脊髄空洞も消失していた。後頭蓋窩腫瘍に脊髄空洞が合併することは稀に報告されており，その空洞発生機序について若干の文献的考察を加え報告した。

A 28-year-old male was admitted to our hospital with the complaints of numbness of the left upper limb and gait disturbance. Neurological examina-tion disclosed slight left dysmetria, truncal ataxia and sensory disturbance at the dermatome of C8 and Th1. CT and MRI scans showed a large mass lesion in the left posterior fossa, ventricular dilatation, cavum septi pellucidi and cavum Vergae, empty sella, cervical syringomyelia and left tonsilar her-niation. The tumor which attatched to cerebellar tentorium was totally removed and a histological diagnosis of meningothelial meningioma was made.
