　アルツハイマー型痴呆（AD）55例と脳血管性痴呆（VD）36例（多発小梗塞型またはBinswanger型）を対象として，SPECTとMRIの画像所見に基づく計量的鑑別診断を試みた。SPECTによる脳血流分布パターンとMRIによる深部白質梗塞病変，側頭葉内側萎縮の各所見を0〜3の4段階にスコア化して評価した結果，AD群とVD群では明らかな相違がみられ，これらは有用な鑑別点と考えられた。次に，数量化理論2類に基づき各スコアに重みづけを加えた集計から，両群の判別を行った。正診率はADで91％（50／55例），VDで89％（32／36例）であり，判別率は90％（82／91例）となった。以上より，SPECTとMRI所見から，ADとVDは高率に鑑別可能となり，本法は臨床的に活用できる有用な方法として期待された。

A new method for differential diagnosis of Alz-heimer-type dementia (AD) and vascular dementia (VD) based on neuroimaging studies was developed by Hayashi's quantification theory II.

Fifty-five patients with AD and 36 patients with VD underwent both SPECT and MRI studies. Both images in each subject were scored according to theextent of hypoperfusion in areas including frontal and temporoparietal regions and the severity of deep white matter lesions and medial temporal lobe atrophy.
