　患者は同胞4人中の2姉妹（1人は剖検）で，両親は血族結婚である。2例はそれぞれ32歳と36歳ころに不随意運動と歩行障害で発病した。剖検例（姉）は初期から四肢・体幹・顔面に及ぶ舞踏病アテトーゼ様の不随意運動と小脳症状を認めた。腱反射は下肢優位に亢進して病的反射は陽性。病期半ばころから痴呆が始まり，末期には筋萎縮，てんかん発作，自律神経異常を認めた。画像診断：小脳は萎縮しているが，脳幹は保たれている。大脳白質はびまん性に軽度低吸収。病理所見：脳重量は1,010g。大脳では白質にびまん性に髄鞘の淡明化をみる。尾状核頭部と被殻尾側は萎縮・変性している。小脳皮質3層と下オリーブ核に変性あり。橋底部は保たれる。脊髄は前角が軽度に変性している以外に異常はない。卵巣萎縮と全身小血管の平滑筋の変性を伴う動脈硬化をみる。本例に類似する剖検例はHolmes型小脳皮質萎縮症として報告されているが，そもそもHolmes型とされた従来の報告例には種々の疾患が混在していたという点に問題がある。

We present two siblings with hereditary corti-cal cerebellar atrophy (CCA) , who showed peculiar clinical features. Their unaffected parents are cousins. The mode of inheritance in this family was autosomal recessive. Both patients developed invol-untary movement and ataxia during the fourth decade. The proband (patient 1) was the elder sister. She developed choreoathetoid involuntary movement and cerebellar ataxia at the age of 32. At the age of 39, she showed mental deterioration and marked gait disturbance due to severe ataxia and amyotrophy.
