天幕下腫瘍を疑つた後頭蓋窩の巨大動脈瘤の1例

A Case of a Large Aneurysm in Posterior Fossa like a Subtentrial Tumor

I．はじめに

　後頭蓋寓動脈瘤は比較的まれな疾患である。頭蓋内動脈瘤で椎骨脳底動脈領域に発生する率はMcDonald　＆ Korb （1939）の報告では18％，Duvoisin　＆　Yahr （1965）では8％，佐野（1966）では5％で，だいたい10〜15％である。しかしながら，頭蓋内動脈瘤の大きさに関しては，本邦の報告例で最大径3cmをこえるものは非常に少なく，内頸動脈領域では，吉村（1959），岡本（1964），時任（1966），柴崎（1967）らの巨大動脈瘤の報告があるが，脳底椎骨動脈領域では吉村・太田（1964）らの剖検による報告をみるのみである。

　今回，われわれは11歳の女児に天幕下腫瘍を疑い諸検査の結果，右後下小脳動脈の巨人動脈瘤を経験し，積極的に後頭下開頭術を行ない，動脈瘤を摘出したので報告する。

Eleven-year-old girl was admitted to the first Department of Surgery of Kagoshima University Hospital with the chief complaints of heavy head-ache, vomiting, and disturbance in the gait. In the neurologic examinations on admission, the patient staggered to right in the vestibular test, and all deep tendon reflexes were hyperactive, but no pathologi-cal reflexes were noticed. Skull X-ray series, ven-triculogram, carotid angiogram, revealed the typical findings like in the subtentrial tumor. The right vertebral angiography revealed 4. 5 × 4. 2 × 3. 2 cm sized aneurysm of right inferior posterior cerebellar artery in the posterior fossa. This aneurysm was resected subtotally following suboccipital craniotomy.

Bibliographically this aneurysm seemed to be the largest one of the aneurysms in the posterior fossa, previously reported in Japan.
