近年になつて頭蓋骨底殊に大後頭孔・上部頸椎附近の骨性並びに中枢神経性畸型が一般臨床家の注目を引く様になつた。即ち扁平頭底(Platy-basia, basilar impression, Atlasassimilation,Occipitalisation)及びArnold-Chiari malfor-mation等で欧米では幾多の治験例の報告が見られ,いずれも決して稀な疾患ではないとされている。この中で中枢神経性畸型であるArnold-Chi-ari malformationに関し本邦に於ては2〜3の報告を散見するに過ぎず,尚未だ屡々看過されている様な状態と思われるので最近我々の教室で経験し手術により軽快せしめた1症例を中心にいささか考察を加えここに報告する次第である。
The Arnold-Chiari malformation is a disease caused by downward migration or elongation and descendence of the cerebellar tonsils and medulla oblongata. Its symptoms are mani-fold and this is by no means rare disease. However, only a few cases have been report-ed in Japan, so we are reporting our one case with some discussion.
Patient is 45 years-oldmale, with chief com-plaints of headache, left tinnitus, right deaf-ness, disturbance of gait and flushed face.
We suspected right acoustic tumor from above mentioned symptoms, though preopera-tively platybasia and basilar impression were proved by x-ray. Suboccipital craniotomy revealed Arnold-Chiari malformation.
