In 1943, Foerster made a comparison between resecting area 4 of the cortex and cutting the pyramidal way in the medulla oblongata and found the fact that spastic paralysis never appeared in the latter one. From this fact, he said that the appearance of spastic paralysis is caused because there is a disturbance in the pyramidal road and also in the extrapyra-midal one, too. And recognized that there is a extrapyramidal center in area 4.

We recently experienced a man, 33 years old, who received a trauma on the left head. He had coplete lost of motility and sensibility of right extremities and a motor aphasia. A tre-panation was performed a month after the trauma and a encephalomalacial cyste caused after brain bleeding was found in the vinicity of the left capsula interna. It's content was removed. After operation a light spastic paralysis remained, but other motile paralysis and symptoms showed a great improvment.

To see it's effect, an electromyogram was taken before and after the operation and a fine observation was made on the recovery of the motile paralysis. Before the operation, a flaccid paralysis was seen in the right extre-mities with motor aphasia and slight sensible paralysis. In it's electromyogram no current appeared in it's passive movement. But after operation, first, the muscle spasmus appeared in the lower extremities with voluntary move-ment. Next this appeared in the upper ex-tremities. The recovery of the motile function was also proved by it's electromyogram.

From this experience an injury in the vincity of the capsula interna shows that it concerns not only the pyramidal road, but also the extra-pyramidal one.
