　当センターに搬送された内因性疾患による院外心肺停止患者を対象に，standard-dose epinephrine（SDE）とhigh-dose epinephrine（HDE）の自己心拍再開率，蘇生成功率および神経学的予後などについて比較検討した．SDE群はエピネフリン1mg,HDE群はエピネフリン5mgをそれぞれ自己心拍が再開するまで5分間隔に投与した．自己心拍再開率，蘇生成功率，神経学的予後はいずれも両群間に有意差はなく，蘇生成功率はSDE群のほうが高い傾向にあった．動物実験では認められているHDEの有効性が本研究では認められなかった原因として，虚血性心疾患の存在，エピネフリンのβ₁作用による心筋酸素消費量の増加などが考えられた．

We compared high-dose epinephrine (HDE) with standard-dose epinephrine (SDE) in improving the outcome of resuscitation after out-of-hospital car-diopulmonary arrest. The SDE group received epine-phrine 1mg, and the HDE group received epinephrine 5mg respectively every 5 minutes until the return of spontaneous circulation. There were no significant differences between the SDE group and the HDE group in the rate of return of spontaneous circulation, rate of successful resuscitation and neurologic outcome. How-ever, the rate of successful resuscitation tended to be higher in the SDE group.
