　症例1：81歳，男性．左下腿の筋肉内血腫を訴え，当科に入院．腹部MRIで最大径14cmの真性大動脈瘤が認められ，血小板数，血漿フィブリノーゲン濃度の減少，FDPの上昇があり，汎播種性血管内凝固（DIC）を合併していた．ヘパリン投与後に人工血管置換術を施行し，DICは軽快した．症例2：60歳，男性．腰痛と吐血があり当科に入院．腹部CTで最大径5.5cmの真性大動脈瘤が認められ，血小板数減少，血漿FDP上昇などがありDICを合併していた．入院2日目に動脈瘤破裂で突然死亡した．当科で経験したDIC非合併例（9例）ではトロンビン・アンチトロンビンⅢ複合体，α₂プラスミンインヒビター・プラスミン複合体，FDP-Dダイマーがほとんどの例で高値であった．以上のことから，腹部真性動脈瘤患者では一般に凝固線溶系が活性化状態にあり，動脈瘤径が著しく大きい例や切迫破裂例ではDICへ進展する可能性があると考えられた．

Two cases of abdominal true aortic aneurysm (AAA) associated with disseminated intravascular coagulation (DIC) were reported. Case 1 was an 81-year-old male who was admitted because of hematoma on the left leg and in whom was found by MRI an aortic aneurysm of 14 cm in diameter. Coagulation studies indicated DIC by revealing thrombocytopenia, hypofibrinogenemia and increased level of FDP. DIC was well controlled by surgical repair of the aneurysm after the administration of a small dose of heparin.
