慢性肺塞栓症に狭心症を合併した1例

A Case of Chronic Recurrent Pulmonary Embolism Complicated with Effort Angina Pectoris

　症例は64歳男性．前胸部圧迫感と息切れを主訴として来院．心電図異常，運動負荷試験および負荷タリウムシンチグラムで陽性所見を認めたため，心臓カテーテル検査施行．肺動脈圧74／21（38）と著しい肺高血圧と冠動脈造影でseg 1の完全閉塞（collateralでseg 2以降が造影された）とseg 7の75％狭窄を認めた．肺動脈造影と肺血流シンチグラムで多発性肺塞栓が判明した．下肢静脈造影で血栓が確認されたことから，二次的な肺高血圧症に進展した慢性反復性肺塞栓症に狭心症を合併したものと診断．肺高血圧の可逆性を検討するため亜硝酸剤とカルシウム拮抗剤による血行動態への影響を調べたところ，急性効果を認めた．従来より肺塞栓症は診断困難な疾患であり，特に本症例では心筋虚血が臨床症状・心電図による診断を困難にしていた．本症例のように血管拡張療法で急性効果を認める例では内科療法の適応があり，狭心症の合併からも妥当な治療法と考えた．

A 50-year-old man was admitted to our hospital because of shortness of breath and an oppresive feeling in his chest on exertion. As a result of examinations including pulmonaryangiography, pulmonary perfusion scan and phlebogram, we diagnosed the case as chronic recurrent pulmonary embolism due to deep vein throm-bosis. Later on, the patient showed an abnormal reac-tion on threadmil exercise test, and revealed redistribu-tion at the anteroseptal and the inferior wall on thalium exercise test.
