運動負荷試験中の心停止，洞房ブロック，洞停止，房室ブロックは非常に稀で実例の呈示は少ない．症例は69歳男性で，狭心症の診断にてtreadmill testを施行し，負荷終了後2分30秒より徐脈さらに発作性完全房室ブロックとなり心停止した．直ちに心臓マッサージを行い洞調律に回復した．冠動脈造影では一枝病変で右冠動脈segment2に90％狭窄を認めた．この病変に対しPTCA（Percutaneous Transluminal Coronary Angioplasty）を施行し残存狭窄47％に改善した．PTCA前後に施行した201Tl負荷心筋シンチグラフィーでも虚血の改善を認めた．PTCA後のtreadmill testでは前回のような不整脈は認められなかった．本例の運動負荷による心停止の機序としては，右冠動脈病変と運動後の迷走神経の過度の興奮関与が疑われ考察を加えて報告した．
We reported a case of angina pectoris with cardiac arrest immediately after treadmill exercise test, and the effect of PTCA in the same case.
A 69-year-old Japanese male had chest oppression on exertion. Initial treadmill test showed 2 mm ST-seg-ment depression in leads V4~6. Two minutes after exer-cise, he had atrio-ventricular (A-V) block and cardiac arrest with episodes of fainting. He was resuscitated by chest thump.
Coronary angiography showed 90 % stenosis in the right coronary artery (RCA). PTCA for RCA was able to dilate the stenotic lesion.
