下顎骨囊胞摘出術中の心停止が冠攣縮性狭心症によるものと疑われた1症例

A Case of Cardiac Arrest due to Strongly Suspected Coronary Artery Spasms during Mandibular Cystectomy

要旨

　心臓疾患の既往歴がない46歳の男性の下顎骨囊胞摘出術中，手術操作による三叉-迷走神経反射に起因する冠動脈攣縮が心停止を来したと疑われる症例を経験した。口腔外科などの三叉神経領域の手術において，原因不明の心停止の際は三叉-迷走神経反射による冠動脈攣縮を検討し，早急に対応する必要がある。

　A 46-year-old man with no previous medical history underwent mandibular cystectomy. During surgery, he was suspected to have developed coronary artery spasms, which may have been caused by oral manipulation, resulting in cardiac arrest. Anesthesia management during oral surgery should be conducted in consideration of possible coronary artery spasms caused by the trigeminal-vagus reflex.
