　有機硝酸エステル剤は末梢血管平滑筋に直接作用し，これを拡張することによって，心臓に対する前負荷および後負荷を減ずる。したがってうっ血性心不全の治療に極めて有用であるが，かような薬剤は経皮的に用いても充分な効果が期待できるところから，ニトログリセリン軟膏（NGO）などが開発され^1,2），長期にわたる心不全の管理に用いられるようになった。

　我が国において新しく開発された硝酸イソソルビドテーブ（TY−0081）は，ポリエステル系フィルムをベースにした粘着テープに有機硝酸エステル剤であるisosor—bide dinitrate （ISDN）を含有させたもので，NGO同様薬剤は皮膚より吸収され，肝臓での代謝をうけることなく体循環に入り，有効血中濃度が長時間維持できるよう工夫されている^3）。

Effects of isosorbide dinitrate tape (TY-0081) were studied in 49 patients with congestive heart failure who were admitted to 25 medical institutions. TY-0081 significantly lowered the heart rate, mean blood pressure, pulmonary vascular resistance and the double product, as well as, reducing the pul-monary capillary wedge pressure and the right atrium pressure after its application, and the effects continued for 24 hours. Serum concentration of ISDN reached to a peak 12 hours after application. Clinical improvement was observed in 67.3% of the cases. It was concluded that TY-0081 was useful for long-term treatment of heart failure and also the combined use of sublingual ISDN proved to be effective, complementing the slow onset of the effects of TY-0081.
