要旨●胃神経内分泌腫瘍（NET）は発生環境の違いによりI〜III型の3つの亜型に分類される．近年，いずれの亜型とも異なる胃壁細胞の機能不全によるIV型胃NETの症例が報告されている．今回，その臨床的特徴を明らかにすることを目的とした．当科でIV型胃NETと診断した1例とI型胃NETと診断した13例を対象とし，臨床背景，画像所見，採血データおよび内視鏡治療後標本の病理組織学的所見を比較した．IV型胃NET症例では，プロトンポンプのαサブユニットをコードするATP4A遺伝子に複合ヘテロ接合体の病的変異が確認された．IV型胃NET症例では胃底腺領域内に10個程度の発赤調の粘膜下腫瘍様隆起を認めていた．背景粘膜は肥厚様であり，I型胃NETでみられるA型胃炎とは明らかに異なっていた．I型胃NETとの臨床徴候の比較では，発症時年齢やガストリン値はほぼ同等であったが，抗胃壁細胞抗体と抗内因子抗体は陰性であることと，MCVと血清ビタミンB₁₂は正常であることが異なっていた．IV型胃NETは，プロトンポンプの機能不全に起因する遺伝性疾患と考えられ，新規の疾患概念である．

　Gastric NET（neuroendocrine tumor）is categorized into types 1 to 3 based on the developmental environment. A case of type 4 gastric NET with parietal cell dysfunction, which is different from any of the subtypes, has been recently reported. This study aimed to clarify the clinical characteristics of type 4 gastric NET. We compared clinical background, imaging findings, blood sampling data, and histopathological results of endoscopically treated specimens of one case of type 4 gastric NET and 13 cases of type 1 gastric NET in Kyushu University Hospital. A compound heterozygous mutation in the ATP4A gene was identified in the patient with type 4 gastric NET. Approximately 10 erythematous submucosal tumor-like raised lesions were observed in the fundic gland region in type 4 gastric NET. The background mucosa was thickened, different from type A gastritis. A comparison of clinical characteristics with type 1 gastric NETs revealed similar age at onset and serum gastrin levels, but with negative antiparietal cell antibody and intrinsic factor antibody, and normal mean corpuscular volume and serum vitamin B₁₂. Gastric NET type 4 is a novel disease entity that is an inherited disorder caused by a proton pump dysfunction.
