Classification of Lymphoproliferative Disorders of the Gastrointestinal Tract

要旨●リンパ腫は最も頻度の高い血液・リンパ系腫瘍であり，国内新規罹患数が年々増加している．リンパ腫の病型・疾患単位（type/entity）分類は，いくつもの混乱を乗り越え，現在のWHO分類に至っている．WHO分類は，第3版以降，一貫してリンパ系腫瘍を細胞系列によって区分し，それぞれのカテゴリーの中に細胞・組織形態，免疫学的表現型，染色体・分子異常，臨床病態のすべてにより定義される病型をリスト化している．その最新版にあたる第5版（2022年公表）では，新たな病型の追加や名称変更などがなされている．今後も，新たな科学的根拠が得られるたびに新しい疾患概念や病型が提唱されることが予想されるが，分類の基本理念に大きな揺らぎはない．臨床医も病理医も，分類やその歴史的背景を理解し，適切に運用することが重要である．

　Lymphoma is the most common hematolymphoid tumor, and its incidence is increasing yearly in Japan. The classification of lymphoma has been standardized in accordance with the current World Health Organization classification. Since the 3rd edition, lymphoid tumors are classified by cell lineage, and each category covers the disease types/entities defined by classical morphology, immunophenotypes, chromosomal and molecular abnormalities, and clinical manifestations. In the 5th edition（published in 2022）, new disease types/entities are added, and some terminologies are renamed. In the future, new disease concepts and disease types/entities should be proposed whenever new evidence is obtained. However, there is no significant change in the basic principles of classification. Therefore, it is important for both clinicians and pathologists to understand the classifications of lymphoid tumors and their historical background.
