要旨●現在，潰瘍性大腸炎を評価するための内視鏡スコアは多数存在するが，スコアの統一化についての一定のコンセンサスはない．これまでのスコアは検者内・検者間一致率などの信頼性評価が十分ではなかったことがその要因の一つである．2012年に提唱されたUCEISはその問題を指摘し検討した上で作成された内視鏡スコアである．今回，筆者らは現在汎用されているMESとUCEISの両者に関して検者間一致率を検討した．その結果，UCEISはMESと比較し，検者間一致率が高い傾向にあった．今後の課題として，UCEISによる粘膜治癒の定義や長期予後との検討などが挙げられるが，妥当性の検証結果からは今後の臨床研究などで使用するスコアとしてより適したものであると考えられた．

　Today, multiple endoscopic scoring systems exist for the evaluation of ulcerative colitis ; however, there is no consensus on the standardization of these scores. One of the reasons for this nonstandardization is the lack of sufficient examination of intra- and intertester consistencies when evaluating system reliability. In response to criticism and out of consideration of this problem, UCEIS（the Ulcerative Colitis Endoscopic Index of Severity）, a scoring system to assess endoscopic severity, was proposed in 2012. Here, we examined the intertester consistency between the currently widely used MES（Mayo endoscopic subscore）and UCEIS. The findings indicated that UCEIS had a higher intertester consistency than that of MES. Future objectives may include the examination of the definitions of mucosal healing in UCEIS and their long-term prognoses. Overall, reliability testing suggested that UCEIS is a more effective scoring system to be used for future clinical research.
