Transparent Hood and Endocuff

要旨●大腸内視鏡施行時にスコープ先端に透明フードを装着することにより視野の確保や処置が容易となり，ポリープの検出率が向上し，大腸内視鏡の盲腸までの挿入率向上や挿入時間の短縮にも有用である．また，エンドカフを装着した場合は，柔らかいフラップがスコープの抜去時にひだにかかるため，ひだの口側の観察しづらい部位や屈曲部の口側でもひだを押さえ込んでめくりながら観察したり，S状結腸などではひだを押し広げて伸ばしたりしながら観察することが可能である．腫瘍検出率やポリープ発見数の向上に有用であるが，盲腸までの挿入率や挿入時間の向上はみられない．重篤な偶発症はみられていないが，フラップによる軽度の粘膜損傷が報告がされている．

　Attachment of a transparent hood to the tip of the colonoscope facilitates maintenance of the visual field and treatment. It has improved not only the adenoma detection rate but also the cecal intubation rate and time. The soft wings of the Endocuff, which hook to the fold during retrieval of the colonoscope, facilitate the observation of the oral side of the fold, thus helping in straightening the sigmoid colon and improving the adenoma detection rate. However, the cecal intubation time and rate was not improved. It had no severe adverse effects, except a minor mucosal injury.
