Characteristics of Gastric Ulcer in a Helicobacter heilmannii-positive

要旨●国民総除菌時代となり，H. pylori感染症に隠れ見逃されてきた胃感染症のひとつにH. heilmannii（ハイルマニイ菌）感染症がある．この菌は，HHLO，NHPHなどの呼称があり，いくつかの種から成るが，ヒトにおいてはその主体はH. suisと，狭義のH. heilmanniiである．人獣共通感染症でありイヌ，ネコなどの伴侶動物（ペット）およびブタが自然宿主と考えられている．ヒトに感染するH. heilmannii（広義）についてはウレアーゼ陰性菌が多いことから，迅速ウレアーゼテスト，尿素呼気試験では陰性あるいは弱陽性となり，H. pyloriに対する抗体にも陰性になることが多い．H. pylori陰性胃潰瘍を含めたH. pylori陰性胃疾患患者のなかにPCR法によるH. heilmannii陽性症例が見出された．その特徴は，軽度の前庭部胃炎を呈する症例が多いことだが，難治性胃潰瘍症例にも認められ，今後の陽性症例の集積，解析が必要である．

　In the era of nationwide Helicobacter pylori eradication, other bacterial infections such as those caused by non-H. pylori Helicobacter（NHPH）have recently attracted attention. NHPH shows a zoonotic infection pattern, and the hosts include pigs, cats, dogs, and other mammals. We present a case of NHPH-positive and H. pylori-negative gastric ulcer during scrutiny in Tokyo. Endoscopy revealed that the ulcer was present near the angle of His, and the neighboring tissues showed mild gastric atrophy. We also found several NHPH-positive cases suffering from gastric MALT lymphoma, nodular gastritis, and duodenal ulcer, and all cases with duodenal ulcer were H. pylori and NHPH positive.

　We have to consider that patients negative or weakly positive for rapid urease test and urea breath test may have NHPH infection.
