要旨●早期胃癌の診断において画像強調観察（IEE）の有用性は高く，範囲診断を中心に普及している．今回当科において2010年1月〜2014年8月までの間にFICEまたはBLI併用拡大観察を施行し，ESD前の術前範囲診断を行った計212病変（FICE：81，BLI：131病変）に関し，範囲診断が困難な症例の特徴を明らかにすることを目標に検討を行った．ESDを施行し，水平断端陽性となった症例は，FICE併用拡大観察において2病変（2.5％）あり，BLI併用拡大観察においては認めなかった．限られた症例数の中では，FICE，BLIで範囲診断困難な症例の特徴には迫ることができなかったが，BLIの登場で早期胃癌に対する拡大観察における微小血管構築像，表面微細構造の描出は格段に向上しており，症例を提示し解説する．

　The preoperative identification of the horizontal extent of an early gastric cancer is important. ME (magnifying endoscopy）has been applied to the diagnosis of early gastric cancer. Recently, in addition to FICE (flexible spectral imaging color enhancement）, which has been used, it came to be able to use BLI (blue laser imaging）. We retrospectively analyzed 212 early gastric cancers from patients who underwent ESD (endoscopic submucosal dissection）after detailed observation by magnifying endoscopy with FICE and BLI. The numbers were 81 and 131, respectively. There was no significant difference between the two groups in horizontal margin free resection, but BLI is considered more useful for observation both microvascular architecture and microsurface structure than FICE.
