6) 拡大内視鏡による癌の進展範囲診断

How to Determine the Horizontal Margin of Early Gastric Cancer by Using a Novel Magnification Endoscopic Technique

要旨　われわれは，微小血管構築像を指標とした早期分化型胃癌の境界診断における拡大内視鏡の有用性を報告してきた．その後，narrow band imaging（NBI）など新しい手法が開発され，新しい知見が集積されてきた．本論文では，拡大内視鏡による境界診断の臨床応用例を呈示し，新しい臨床応用について論述する．要約すると，現在までに報告した知見に加えて，境界診断における新しい応用は，①副病変の発見，②白色光拡大観察での境界診断困難例に対するNBI併用拡大観察による粘膜微小血管構築像や粘膜表面微細構造の有用性，③生検でのみ存在診断が可能な早期胃癌（occult cancer）の診断であった．特に，Uedo らが報告したNBIでのみ観察可能なlight blue crestは，境界診断への新しい有用な指標と考えられた．

　We describe the novel clinical usefulness of magnification endoscopy for determining the horizontal margin of early gastric cancer. In summary, the usefulness was as follows : (1) identification of a daughter lesion, (2) usefulness of magnification endoscopy with narrow band imaging which can clearly visualize both microvascular architecture and microsurface structure, and (3) diagnosis of an occult cancer which could not be identified by non-magnification endoscopy alone. Among these findings, so called "light blue crest", which cannot be visualized by white light imaging alone, but which can be visualized by NBI, could be a new useful makers for determining the margins of carcinoma.
