症例
患 者：M. K. 47歳 女
主 訴：悪寒・戦懐を伴う発熱，やせ，疲労感，下痢，精神不安定
家族歴・既往歴：特記すべきものなし
現病歴：1970年，心窩部痛を生じ，近医を受診，胃透視，胃内視鏡検査の結果，胃潰瘍の診断を受けた．1年半通院治療し心窩部痛は徐々に消失した．1972年夏より下痢をきたすようになり，やせがひどくなってきたので，A病院にて胃透視，胃内視鏡検査などを受けたが，異常は認められなかった．そのまま放置していたが，1973年1月頃，やせがひどくなり，感冒にもかかりやすくなった．同年7月初旬，左腹部痛がはじまり，B病院にて胃透視，その他の検査を受けたが異常は認められなかった．しかし，左腹部痛に加え，食欲不振，全身倦怠感，下痢などが持続したため，さらにC病院へ検査入院した．一般検査および胃透視，イルリゴスコピーなどを行ない，過敏性大腸症候群と診断された．入院時より38℃前後の悪寒戦慄を伴う発熱があり，容易に解熱しなかった．同院では心理的な要素が強いと考え，1973年9月当科外来へ紹介してきた．甲状腺腫大および貧血もあったため，同年9月19日，当科へ入院となる．
The patient was a 47-year-old female, who had been admitted because of fever with shaking chill, emaciation, general fatigue, diarrhea and anxiety.
Radiography showed deformity, shortening of the bowel, string sign, skip lesion, etc. Endoscopy showed pseudopolyps, bleeding and edema in the ascending colon.
These findings and pathological study in biopsy specimens of the ascending colon was more suggestive of Crohn's disease than tuberculosis.
However, this case has been finally diagnosed as intestinal tuberculosis because tubercle bacillus was identified by continuous culture of the stool and operated material.
