前壁Ⅱc型早期胃癌の1例

A Case of Ⅱc Type Early Carcinoma on the Anterior Wall of the Stomach

はじめに

　胃前壁病変のX線診断は，最近，熊倉¹⁾²⁾³⁾をはじめとして，多くの報告^4)～7)がある．この症例も，ルーチン検査として腹臥位二重造影法による胃前壁撮影を行なってきたところ発見されたものである．

　A man aged 36 visited the author's clinic with a complaint of left hypochondrial pain. He had been suffering from increasing gastric distress for the past two years. In the interim he had been repeatedly subjected to x-ray examinations in other clinics, but no definite findings were demonstrated, nor was there any definite abnormality at the initial x-ray study done by the author except in one film of the anterior wall taken in double contrast method. A lesion was demonstrated there as a shallow, en face niche of irregular outline with rugal convergence toward it, suggesting of malignancy.

　The patient was then sent to a university hospital where he was operated on under the diagnosis of gastric cancer as confirmed by both endoscopic and cytological examinations. The resected stomach revealed an early cancer of IIc type on the anterior wall of the stomach.

　This case seems to corroborate the necessity of examining the anterior wall of the stomach in double contrast method even in routine x-ray examination of the stomach.
