X-RAY EXAMINATION METHODS FOR THE LESION ON THE GREATER CURVATURE OF THE STOMACH

Ⅰ．はじめに

　大彎側のX線診断に関する文献は，きわめて少ない．その理由の1つは，大彎側の病変，特に陥凹性病変の頻度が少ないこと，いま1つは，大彎側の病変は悪性が多く，ことに陥凹性病変のほとんどは悪性であり，良性か悪性かの鑑別を，そのつど厳密に行なうまでもないといった観念が支配的であったためと思われる．

　しかし，近年になって，Palmer，Bockusらが大彎側良性潰瘍がまれではないという数値を，常岡が大彎側良性潰瘍のX線，内視鏡所見を，さらに村上らが，大彎側病変に関する詳細な報告を発表し，改めて，大彎側のX線診断上の諸問題を検討する意義や必要性がでてきたわけである．

　大彎側のX線診断の上での問題点を，文献や，われわれの経験から，X線撮影の面，X線読影の面からあげてみると，次のような項目になる．

　X線撮影の面では

　1）確実に病変が拾いあげられる撮影法

　2）病変の性状をよく現わすような撮影法

　X線読影の面では

　1）病変を確実に拾いあげるための読影

　2）正確に質的診断するための読影

は，実際上どのようにしたらよいか，ということになる．

　そこでわれわれは，以上の諸事項について，実際の症例をとり出して検討してみた．

　なお，大彎側病変は，頻度の上では隆起性病変が多いが，隆起性病変については，X線診断の面では，隆起性病変一般の診断理論をそのままあてはめることができ，大彎側病変であるからといった特異性は少ない．そこで今回は，大彎にあるために，良性，悪性の鑑別がより困難である陥凹性病変をとりあげてみた．

　A roentgenological method is described in this paper of visualizing depressed lesion on the greater curvature of the stomach, with special reference to its interpretation.

　To detect and correctly interprete depressed lesion on the greater curve, it is necessary to pay close attention to minute abnormal findings such as depression, protrusion, irregularity of the gastric wall and so forth in X-ray pictures of the stomach in its properly distended state. In this way one is less apt to overlook any lesion thereon than was formerly reported.

　Aspects of depressed lesion on the greater curvature are best delineated by double contrast study with large amount of air, with frequent alteration of patient's position. In addition to this, compression method is also helpful, if the degree and the direction of exerted force is proportioned in moderately barium-filled stomach.

　As to the interpretation of visualized findings there, especially of depressed lesion, much remains to be done for further studies, as it is sometimes dimffcult to differentiate between benign and malignant lesions, because they are visualized as of variegated patterns.

　One of the ways to facilitate their differential diagnosis certainly lies in detailed macroscopical examination of every resected specimen as well as its meticulous correlation with X-ray findings prior to operative surgery.
