　Double pylorusとは，表面を胃粘膜または十二指腸粘膜で覆われた柔らかい索状組織が主としてpylorusの前後壁に橋をかけるように存在していて，これを口側より見た場合pylorusが二分されて，あたかも2つの胃出口があるように見えるところからこの呼称を持つ症候群のことで，pyloric band，double pyloric channelあるいはpyloroduodenal fistulaとも呼ばれている．

　1969年Smith and Tuttle¹⁾によって初めて報告されて以来，今日までに60例の報告をみるが，その成因については，これをcongenital pyloric duplicationの1つの形態であるとする意見^1)～11)と，pyloroduodenal peptic ulcerによって後天的に形成されたものだとする意見^12)～31)とがある．

　A case of double pylorus is reported, with illustrations of the radiological, endoscopical, and histological characteristics. As far as can be ascertained, a complete review of the literature (60 cases) is given. Both possible genetic mechanisms of this formation are discussed in detail; primary or congenital, and secondary or acquired by inflammation.

　After analysis of the histology of the pyloric bands of the hitherto related 12 surgical cases, the opinion is expressed that the so-called double pylorus represents an acquired form of peptic deformity. We think trans-sphincter tunnel formation of the prepyloric ulcer makes the pyloric band, containing gastroduodenal mucosa and smooth muscle.
