要旨　66歳の男性．1980年初旬，食思不振，腹部膨満感を主訴として来院し，血液生化学検査および上部消化管X線検査などが行われ，慢性胃炎および慢性肝炎とされた．初回検査より4年3か月後の1984年11月7日，肝機能悪化を機に行われた第2回上部消化管X線検査により中部食道に27×16mmの表在型食道癌が発見された．1984年12月13日に行われた第3回食道精密X線検査のX線写真から逆追跡したところ，4年3か月前に同じ部位に粘膜のわずかな腫大を示した症例を経験したので報告した．

　A 66-year-old man with chronic hepatitis was first admitted to our clinic on Sept. 13, 1980 complaining of indigestion. An upper gastrointestinal series and other laboratory tests were performed.

　On Nov. 7, 1984 a second upper gastrointestinal series was performed which suggested esophageal abnormalities. A diagnosis of moderately differentiated squamous cell carcinoma of the esophagus, Im, slightly elevated type, was made by endoscopic biopsy on Dec. 13, 1984, and detailed esophageal x-ray (the third) on Dec. 18, 1984.

　The patient underwent surgery on Jan. 22, 1985. It revealed cancerous involvment mainly of the muscularis mucosae, and partial infiltrating some of the submucosal layer. No lymph node metastasis was found. Review of the prior radiographs revealed slight mucosal swelling at the location of the carcinoma which we believe was the early manifestation of the carcinoma.
