病理の立場からみた胃潰瘍の質的診断の限界

The Utmost Limits of Qualitative Diagnosis of Gastric Ulcer Considered from the Standpoint of Pathology

　病理の立場から，胃潰瘍の質的診断とその限界について書くように依頼された．大変難しい課題で，できれば避けて通りたいという気持もないではなかったが，胃疾患の診断や治療にとって依然として重要な課題の一つであり，診断の限界を知り，その限界をふまえてどのように対処していったらよいか，また今後の問題点はどこにあるかなどを得られている成績や知見をもとにご紹介し，あわせて筆者らの考えを述べさせていただく良い機会と思い直すようになった．

　いうまでもなくこの問題の基本には，数年前に病理学者や外科医の間で種々論議され，未だに意見の一致をみるにいたっていない潰瘍癌の組織学的判定の基準の問題がある^1)～11)．この基準は，潰瘍と癌との組織レベルにおける位置的関係を重要視したものであるが，両者の発生後の古さの比較という時間的要素がその背景にあることもご存知の通りである．しかし時代の推移とともに診断に多くの進歩改良が加えられ，切除胃の内容も次第に変ってきており，このような観点から古くて新しいこの問題をこの辺りで考察してみるのも，また大事なことではないかと思う．

　Most important problem for considering the qualitative diagnosis of gastric ulcer seems to be gradual changes in the nature of gastric ulcer and of early cancer in the resected stomachs year by year. Based upon histological examinations and statistical results of the resected stomachs during these 20 years at Yokoyama's Hospital, the changes were summarized in the cases of gastric ulcer as decrease in relative frequency of chronic type and in acute and subacute type and in the cases of early gastric cancer as decrease in relative frequency of Type III (excavated or ulcerated type) and increase in Type II c (eroded type).

　Reappraisal of type III early gastric cancer for its macroscopic diagnosis in 316 cases thus far obtained shows that 65 cases (20.3 per cent) were recorded simply as gastric ulcer. On the other hand, of 6,023 cases of macroscopically diagnosed gastric ulcer, histologic study confirmed in 65 cases (1.1 per cent) cancer around ulcer. Reexamination of their gross pictures indicates the existence of abnormal findings, although slight, around ulcer in 20 cases, where cancer should have been suspected from the outset.

　On the basis of the above-mentioned results, we have referred to the utmost limits of diagnosis in gastric ulcer as well as to those problems that remain to be solved.
