クローン病の検討―内視鏡診断とその基礎

Study on Crohn's Disease, Its Endoscopic Diagnosis and the Fundamentals

　クローン病は日本ではその存在が確実視されるようになってからの歴史が浅く，症例数も絶対数，相対数（腸結核，潰瘍性大腸炎UCに比べて）ともに少ない．したがってその診断には慎重を要する．切除症例について充分な病理組織学的検討をまってはじめて診断するのが理想的である．しかしクローン病は進行が緩徐であり，術後の再発が多いので，外科的切除は最後の手段とし，臨床診断に基づき，まず内科的治療を試みるべき疾患である．それゆえ臨床診断とくに形態学に基づくX線，内視鏡の所見は重要である．欧米には各種の診断基準があり，常頻用されている．日本では主として腸結核の存在により除外規定が多く，診断基準を機械的に適用すると，他疾患の混入が多くなりすぎる．

　現状では各症例を1例1例十分に検討して診断を下すべきである．本稿では切除例5例と，非切除例7例（うち2例は他施設ですでに手術され再発して来院したもの）につき検討した．

　In all diseases, the clinical stage influences a difficulty in diagnosis. Same situation is observed even in Crohn's disease and the clinical diagnosis of it in relatively early stage showing only discrete ulcers, is almost difficult.

　Diagnostic criteria by scoring points have been often used in European literatures, but this method is not useful any more in Japan because of too much regulations to rule out.

　We have to make the diagnosis each case by exact work-up.
