  • Look Inside

On the basis of a personal experience obtained at St. Mark's Hospital in London, radiological and macroscopical appearance of Crohn's ileo-colitis in England is compared with that of Japan. The most striking features of Crohn's ileitis in England are the stenosis of the bowel with marked thickening of the bowel wall and deep fissuring ulcers. Cobblestone appearance is also one of the most characteristic features. In Japan, longitudinal ulcerations on the mesenteric site are the most striking feature and the longitudinal ulcer may be present with or without Crohn's colitis. This is not associated with either stenosis or the thickening of the bowel wall but with aphthoid ulcers. Yet, Crohn's ileitis similar to that of English ileitis also exists in Japan. Although macroscopical appearances are different from those of typically English Crohn's ileitis, histological appearances are identical to those of Crohn's disease. Whether this peculiar type of Japanese Crohn's ileitis with longitudinal ulcer on the mesenteric site is merely an early phase of Crohn's ileitis, or characteristic of Japanese Crohn's ileitis is still to be researched. Therefore, further comparative study is required in this respect. In regard to Crohn's colitis there seems to be no difference in macroscopic appearances between England and Japan.
