コンプライアンスからみた胃潰瘍治療の実態

The Compliance and Healing Rate of Peptic Gastric Ulcer in Long-term Medical Treatment

要旨　胃潰瘍の治癒動態を明らかにし，胃潰瘍治療の現実をコンプライアンスの面から分析するために，1982年から1991年の間に癌研附属病院で5年以上の投薬（＝服薬とみなす）を行った胃潰瘍患者137例について，投薬歴とX線・内視鏡検査歴における潰瘍の治癒・非治癒の関係を時系列的に分析し，以下の結果を得た．①無投薬の期間に行われた検査では，全例において潰瘍は治癒していない確率が有意に高かった．②H₂ブロッカー使用群120例のみにおいても，無投薬の期間に行われた検査で潰瘍が治癒していない確率が有意に高かった．③H₂ブロッカー使用群と他剤使用群における非治癒率を比較すると，H₂ブロッカー使用群における非治癒率が有意に高かった．④全例における非治癒率と非治癒投薬率の関係について有意差検定を行うと，非治癒投薬率が40％未満であっても非治癒率は20％未満にとどまる群と，非治癒投薬率は80％以上の高率が維持されている場合に非治癒率が20～80％の分散を示す群の2群に分かれた．⑤H₂ブロッカー使用群のみにおける非治癒率と非治癒投薬率の関係について有意差検定を行うと，非治癒投薬率が80％以上の高率が維持されている状態で非治癒率は20～80％の分散を示した．⑥胃角部の潰瘍は他の部位の潰瘍に比して非治癒の割合が有意に高かった．⑦胃角部の線状潰瘍には全経過において非治癒と判断される例があったが，検査資料が不十分なため，胃角部の潰瘍の中から線状潰瘍の頻度を明らかにすることはできなかった．⑧嚢状胃を呈する胃角部の線状潰瘍においては，胃排出能の低下がH₂ブロッカーの吸収を阻害する可能性があり，このことが難治要因の1つと考えられた．

　Time sequencial analysis on the correlation of medication and non-healing rate of peptic ulcer in 137 patients who underwent medical treatment for more than 5 years between 1982 and 1991 at the Cancer Institute Hospital.

　In all patients the non-healing rate of the ulcers was significantly higher in the radiographic or endoscopic examination which was done during non-medication period (p＜0.01), and in the patients treated with H₂-RA it was also significantly higher during non-medication period (p＜0.01).

　The non-medication rate was correlated with the non-healing rate of the ulcer, and all patients (n=137) were divided into two groups: the one in which the non-healing rate remained lower than 20% in the nonmedication rate lower than 40% (p＜0.01 and p＜0.05), and the other in which the non-healing rate was varied from 20% to 80% while the non-medication rate was kept more than 80% (p＜0.01 and p＜0.05). In the group of the patients treated with H₂-RA (n=120) the non-healing rate was varied from 20% to 80% while the non-medication rate was kept in more than 80% (p＜0.01 and p＜0.05), and the presence of H₂-RA-resistent ulcer was suggested.

　The non-healing rate was compared between the group of the patients treated by the other medicine than H₂-RA and the group of those treated with H₂-RA. The non-healing rate during non-medication period was significantly higher in the group of those treated with H₂-RA.

　It is considered that the compliance is likely to be discontinuous in the group of the patients treated with H₂-RA which gives a relief from pain in a short period and shortens the time for ulcer healing.
