大腸結核瘢痕部に“DALM”を合併した1例

要旨　症例は，54歳，女性．下血にて近医を受診し，画像所見より腸結核が疑われ抗結核療法を約1年間施行された後，患者の希望により当科紹介となった，注腸X線および内視鏡所見では上行結腸から盲腸にかけて腸管の短縮および変形を認め，ハウストラは消失していた．上行結腸から盲腸にかけて瘢痕萎縮帯を認め，回盲弁は開大していた．上行結腸下部の瘢痕萎縮帯内に丈の低い均一な顆粒状隆起から成るⅡa集簇型病変を認めた．病変の大きさは約7cm長で管腔の全周を占めていた．同病変の拡大内視鏡所見ではⅡ型とⅢ_L型pitの混在する部とシダの葉様（Ⅲ_H型）とⅣ型pitの混在する部分が認められた．腸切除標本では，回腸末端から上行結腸にかけて，多発性の潰瘍瘢痕とびらんの形成がみられ，開放性潰瘍部の粘膜下層には非乾酪性肉芽腫が散見された．上行結腸下部の潰瘍瘢痕帯部に，7.6×5.5cm大のⅡa集簇型病変を認めた．病理組織学的に，同病変は粘膜内に乳頭状に増殖する異型腺管群から成り，異型度の高い部分と異型度が低く再生性あるいは過形成性上皮様の部分が混在するdysplasiaと診断された．すなわち，このⅡa集簇型病変は結核性腸炎に合併したdysplasiaと考えられた．本邦では，大腸結核にdysplasiaがDALMの形態で合併した例は今までに報告されておらず，興味深い症例と考えられた．

　A 54-year-old woman visited a nearby hospital with the complaint of bloody stool. Accoding to the findings of barium enema and colonoscopy, the patient was diagnosed as having intestinal tuberculosis. She was refferred to our hospital because of her request after one year's administration of anti-tuberculosis drugs. The barium enema and colonoscopy revealed shortening of the ascending colon, deformity of the Ileocecal valve, and a wide superficial elevated lesion of Ⅱa-aggregat-ing type. Magnifying colonoscopy revealed type Ⅱ, Ⅲ_L, Ⅲ_H, Ⅳ pit pattern on the Ⅱa-aggregating lesion. The histological examination of the resected colon demonstrated multiple ulcer scars with marked fibrosis and several small open ulcers and non-caseating granuloma in the ascending colon. The Ⅱa-aggregating lesion consisted of atypical glands of low and high grade dysplasia. We thus reported the first reported case of DALM (dysplasia associated lesion or mass), associated with colon tuberculosis.
