Sydney Systemの利点と問題点―臨床の立場から

Advantages and Disadvantages of the Sydney System - from Clinical View Point

要旨　1990年に提唱されたSydney Systemによる胃炎分類の利点と問題点について検討した．当教室員の，従来の内視鏡分類をSydney分類に当てはめると，よく対応した．更に，教室員にSydney分類による内視鏡所見の記載を求めたところ，胃粘膜所見を従来よりも注意深く観察するようになった．以上の結果とHelicobacter Pyloriと各種胃疾患との関連研究の重要性を考え合わせると，Sydney Systemによる胃炎分類は優れていることを認めざるを得ない．いくつかの問題点は残るが，まずは日本においてもSydney Systemを受け入れるべきと考える．

　Advantages and disadvantages of the Sydney System, a gastritis classification system proposed in 1990, were evaluated. Classification based on this system showed good agreement with results of the conventional classification based on endoscopic findings made by our staff members. They have become more attentive to mucosal findings than before since they were asked to record endoscopic findings in accordance with the Sydney System. These findings, coupled with the importance of studies on the relationship between Helicobacter pylori and various gastric diseases, indicate that it is useful to classify gastritis according to the Sydney System. Although this system has some disadvantages, it should be accepted for use in Japan as well.
