要旨　近年，HIV感染患者数の急激な増加が社会問題となっている．このため，日常の診療においてHIV抗体陽性者に対して内視鏡検査をする機会もまれなことではなくなってきている．われわれは，消化管病変を認めたHIV抗体陽性の3症例を経験した．免疫不全による食道カンジダ症，CMV（cytomegalovirus）による食道潰瘍および下血を伴った大腸潰瘍であった．食道カンジダ症は，厚い全周性の白苔を有しており，CMV食道潰瘍は潰瘍底に白苔のない打ち抜き様の潰瘍であった．ともに特徴的な内視鏡像であった．

　Recently in Japan, the number of patients with HIV infection had been growing rapidly, so we sometimes perform endoscopic examination for patients with HIV infection. We have encountered three cases of AIDS-related gastrointestinal diseases. AIDS-related lesions usually have characteristic endoscopic findings. Candida esophagitis presents as thick white plaques on the esophageal wall. The ulcer caused by cytomegalovirus presents as a punched out ulcer.
